This policy explains how The Complete Athlete Performance Therapy handles information submitted through this website, tcatherapy.com. It covers the website only. Your clinical record, once you are a patient, is handled separately under the privacy practices you are given at the clinic.
What the form collects
The booking form on this site asks for five things:
- Your name
- Your phone number
- Your email address
- What brings you in, in your own words
- The time of day that is best to reach you
Nothing else is required, and there is no hidden field collecting anything you have not typed. Please do not send detailed medical history through the form. A short line about what hurts is all we need to call you back.
What we do with it
We use it to contact you about care: to return your enquiry, to arrange or confirm an appointment, and to answer the question you asked. That is the whole purpose.
We do not sell, rent or trade your information. We do not pass it to advertisers or list brokers. We share it only where we have to in order to run the practice or where the law requires it.
Where it is stored
This website is hosted on Netlify, and form submissions are captured and stored by Netlify Forms before they reach us. Your submission is subject to Netlify's own handling of the data it processes on our behalf.
Two other things load on this site from outside providers: a Google reviews display, served by Elfsight, which shows public reviews left on our Google Business Profile, and web fonts served by Google Fonts. When your browser loads those, it contacts those providers directly, as it does with any embedded content anywhere on the web. Neither one receives anything you type into our form.
How to have your information removed
Ask us and we will delete it. Email andrew@tcatherapy.com or call (210) 890-2411 and say you would like your enquiry deleted. You can also ask us for a copy of what we hold, or ask us to correct something that is wrong. There is no charge and you do not need a reason.
Changes to this policy
If we change how any of the above works, we will change this page and update the date at the top of it. There is no other version of this policy in force.
